Prescribed Appliance Form

Any player wearing a protective cast or medical device will be required to submit a form signed by a Physician, the player's Parent of Guardian and the player's Head Coach. This form must be completey filled out, signed by all required parties and made available before any child will be allowed to participate

Any player who wishes to transfer leagues from one season to another season must file a "Player Release Form" to the League Director of the present league.
